In response to the recent opioid crisis, the Indiana State Department of Health developed an initiative called Safety PIN (Protecting Indiana’s Newborns) to reduce the infant mortality rate. As part of this initiative, Choices Coordinated Care Solutions provides care coordination services to five counties in Southeastern Indiana to help pregnant women, recent mothers (to children under one year of age), and expecting fathers or recent fathers.

Care Coordinators are supporting clients to access to key services to improve the social determinants of health, including healthcare, education, and support (financial, emotional, and social). This improved access supports the health of the mother and baby by reducing the impact of typical causes of infant mortality.
