The Choices Coordinated Care Solutions Provider Network is a vehicle for providing a wide array of community-based, individualized services to children and families with complex emotional and behavioral needs. An integral part of cross-system care coordination is the development of a broad network of providers, including both formal and informal supports.
For decades, Choices has utilized multiple strategies to develop provider networks. Our approach has evolved to address the needs of the youth and families, to focus on systemic challenges, and grow services across communities. We have learned several key lessons through this experience.
